In the unfair trial of Navid Afkari, Sadati has a direct responsibility to disregard the defendant's allegations of severe torture in order to obtain false confessions, as well as to issuing a conviction sentence without substantial evidence against the defendant other than false confessions obtained under torture. Navid Afkari was arrested during the August 2018 protests in Shiraz. He was then charged with “Moharebeh” (armed struggle against the Islamic System) and “insulting the supreme leader2 in Branch 1 of the Revolutionary Court of Shiraz, headed by judge Sadati, and also with the murder of a security officer in the provincial criminal court presided by Mohsen Rajaeinia, Vahid Asadi and Mehrdad Tahmtan.
However, no evidence was provided for his conviction other than his own confession. Navid Afkari had repeatedly spoken in the court and through audio messages from prison about severe torture imposed on him to obtain confessions. One of the detainees in the Shiraz Criminal Police Detention Centre also testified about the torture of Navid Afkari. Eventually, on the morning of September 12, 2020, Navid Afkari was secretly executed in Shiraz without going through the legal process, without informing his lawyer and family, and without the right to a last visit. His body was buried at night in his native village without permission for his family to see his body, except for his face. His family and witnesses testified about the effects of the beatings and fractures on his face during the funeral.

On 24 September 2020, the United States sanctioned several Iranian officials and entities, including Mahmoud Sadati, for gross violations of human rights. According to the statement issued by the Secretary of State, Mohmoud Sadati is"reported to be involved in the appalling case of Navid Afkari, a young athlete who was imprisoned and brutally executed on September 12, 2020, by Iranian authorities." In accordance with these sanctions, Sadati is banned from entering the US and his assets in the US or in control of American banks and companies will be frozen.
